Project Description
True story - my parents own a tattoo parlor back in Indiana. Part of the guidelines mandated by Indiana Health Department are to keep very detailed record about each tattoo they apply.
Most customer had trouble filling out the original form, which led to records with missing or incomplete information. Using the original form and the guidelines from the Health Department as a starting point, I completely redesigned the form from scratch.
The result? Customers now fill out the form correctly nearly 100% of the time, which means my parent’s record keeping is now up to snuff. Read the related post for more information on the process behind designing this form.
